Job summary

Encore Village of Foulk is seeking an Occupational Therapist to organize and conduct therapy programs across Skilled and Outpatient Departments, guiding patient participation to restore and enhance performance.

You will screen, evaluate, and treat patients, collaborate with the rehabilitation team, supervise OT assistants, and ensure documentation and safety compliance. A DE state license and CPR are required; strong communication and problem-solving skills are essential.

Qualifications

• Graduate of an accredited Occupational Therapist (OT) therapy program.
• Current DE state licensure.
• CPR Certification.
• Excellent verbal, written and interpersonal communication skills.
• Results-oriented with strong critical thinking and problem-solving skills.
• Proficient with therapy documentation (NetHealth).

Responsibilities

• Supervise Occupational Therapy Assistants, temporary licensees, students, and support personnel per licensure and standards.
• Deliver rehab services per established plan of care; adhere to schedules and document services.
• Attend and contribute to patient care conferences and related meetings.
• Refer to other disciplines and agencies to address patient needs.
• Consult with rehab team to select appropriate therapies.
• Adapt plan of care to individual patient needs.
• Promote a safe environment and enforce safety measures.
• Order supplies and equipment as necessary.
• Maintain and report on equipment condition and repairs.
• Provide patient-centered care prioritizing high-quality service.
• Screen, examine, and evaluate patients; develop treatment goals and implement plans.
• Instruct patients in activities and progress monitoring; reexamine as needed.
