Job summary

Encore Village of Foulk is seeking an Occupational Therapist to organize and conduct therapy programs across Skilled and Outpatient Departments, aiding rehabilitation and patient independence.

The OT will supervise assistants, collaborate with the rehab team, and tailor plans of care to each patient while maintaining accurate documentation and ensuring a safe, compassionate environment. Delaware licensure and CPR are required.

Qualifications

• Graduate of an accredited OT therapy program.
• Current Delaware state licensure.
• CPR Certification.
• Excellent verbal, written and interpersonal communication skills.
• Results-oriented with strong critical thinking and problem-solving skills.
• Proficient with therapy documentation (NetHealth).

Responsibilities

• Supervise OT Assistants, temporary licensees, students, and support personnel per licensure and standards.
• Deliver rehab services per care plans and patient schedules with timely documentation.
• Attend patient care, staffing conferences and related meetings.
• Make referrals to other disciplines and agencies for patient needs.
• Consult with rehab team to select appropriate therapies for each patient.
• Adapt plans of care to individual patient needs.
• Promote a safe environment and eliminate hazards.
• Order supplies and equipment as necessary.
• Maintain and report equipment status and repairs.
• Prioritize patient service with compassionate care.
• Screen, examine, and evaluate patients; establish diagnoses and goals; plan and document treatment.
• Design, fabricate, fit adaptive devices (orthotics/prosthetics).
• Select activities suited to patient capacity.
• Instruct patients in functional activities and monitor progress.
• Perform routine reexaminations to modify plans.
• Perform other related duties.
