Job summary

Encore at Foulk is currently hiring for a PRN Occupational Therapist to organize and conduct occupational therapy programs for the facility. The OT will work in both Skilled and Outpatient Departments and guide patient participation to restore and enhance performance.

Responsibilities include delivering rehab services, attending care conferences, making referrals, adapting plans, and ensuring patient safety and documentation compliance.

Qualifications

• Graduate of an accredited Occupational Therapist (OT) therapy program.
• Current DE state licensure.
• CPR Certification.
• Excellent verbal, written and interpersonal communication skills.

Responsibilities

• Deliver rehab services effectively and efficiently; follow plan of care and schedule.
• Attend patient care and staffing conferences; contribute to related meetings.
• Refer to other disciplines and agencies to meet patient needs.
• Collaborate with rehabilitation team to select appropriate therapeutic regimens.
