Qualifications

• Current MA State License as an Occupational Therapist.
• Must have flexibility to be part of a dynamic and cooperative team, to distribute workload equitably and according to staff position within the department.
• Ability to evaluate or assess patients, participate in consultations, teaching and meetings with residents, staff, family, and other professionals to plan treatment and discharge plans and to educate patients and staff.

Responsibilities

• Provide occupational therapy services to the facility promoting rehab excellence and interdisciplinary cooperation.
• Evaluate patients in a timely fashion as required by state, payor source or facility requirements.
• Screen patients at admission or as requested by director, promptly.
• Develop plans of treatment and goals for patients.
• Provide treatment as indicated by the plan after evaluation.
• Communicate with other disciplines and interact with staff as needed per treatment plan.
• Provide routine documentation per Medicare, facility, and payor guidelines.
• Participate in family conferences when requested.
• Record and submit all required documentation and charges timely.

Qualifications for Occupational Therapist:

• Current MA State License as a Occupational Therapist
• Must have flexibility as a person to be a part of a dynamic and cooperative team, to distribute the workload equitably and according to the individual’s staff position within the department.
• Have the capability to evaluate or assess patients, participate in consultations, teaching and meeting with all levels of residents, staff, family, and other professionals, in order to: plan individualized treatment plans and goals, discuss and plan appropriate discharge plans, and teach and counsel patients and staff

Responsibilities for Occupational Therapist:

• Providing occupational therapy services to the facility promoting excellence of rehab care, promoting interdisciplinary cooperation and quality of patients care and promoting the facility geriatric rehab program to the medical and public.
• Evaluate patients within timely fashion, as required by state, payor source or facility requirements
• Screen patients upon admission and/or when requested by director, within a timely fashion
• Develops appropriate plan of treatment and goals for the patient
• Provides treatment as indicated by plan of treatment developed after evaluation
• Communicate with other disciplines and interacts with staff as necessary, as indicated by the treatment plan
• Provide routine documentation according to Medicare, facility, and other payor source guidelines
• Participate in family conferences when requested
• Record and submit in timely fashion all required documentation and treatment charges, according to policy and procedure
