Job summary

Stellar Health Group is seeking a licensed Occupational Therapist for weekend per diem coverage. The role emphasizes a collaborative, family-like therapy team and engaged patient care.

Applicants should hold a Massachusetts OT license and demonstrate flexibility to share workload within a dynamic department. Responsibilities include evaluating patients, developing individual treatment plans, and documenting care in line with Medicare and other payors.

Qualifications

• Current MA State License as a Occupational Therapist.
• Must have flexibility to be part of a dynamic and cooperative team, to distribute workload equitably.
• Ability to evaluate or assess patients, participate in consultations, teaching and meetings with residents, staff, family and other professionals.

Responsibilities

• Provide occupational therapy services to the facility to promote rehab care quality and interdisciplinary cooperation.
• Evaluate patients promptly as required by state, payer source or facility rules.
• Screen patients at admission or on request by director, promptly and efficiently.
• Develop plans of treatment and goals for each patient.
• Provide treatments as indicated by the plan of care.
• Document as required by Medicare and other payors in a timely manner.
• Participate in family conferences when requested.

Qualifications for Occupational Therapist:

• Current MA State License as a Occupational Therapist
• Must have flexibility as a person to be a part of a dynamic and cooperative team, to distribute the workload equitably and according to the individual’s staff position within the department.
• Have the capability to evaluate or assess patients, participate in consultations, teaching and meeting with all levels of residents, staff, family, and other professionals, in order to: plan individualized treatment plans and goals, discuss and plan appropriate discharge plans, and teach and counsel patients and staff

Responsibilities for Occupational Therapist:

• Providing occupational therapy services to the facility promoting excellence of rehab care, promoting interdisciplinary cooperation and quality of patients care and promoting the facility geriatric rehab program to the medical and public.
• Evaluate patients within timely fashion, as required by state, payor source or facility requirements
• Screen patients upon admission and/or when requested by director, within a timely fashion
• Develops appropriate plan of treatment and goals for the patient
• Provides treatment as indicated by plan of treatment developed after evaluation
• Communicate with other disciplines and interacts with staff as necessary, as indicated by the treatment plan
• Provide routine documentation according to Medicare, facility, and other payor source guidelines
• Participate in family conferences when requested
• Record and submit in timely fashion all required documentation and treatment charges, according to policy and procedure
