Job summary

One Home Health in Indiana is seeking a part-time licensed Occupational Therapist to join our dedicated team serving Lake and Porter counties and surrounding areas. You will perform in-home evaluations, design treatment plans, and collaborate with an interdisciplinary team to support patient independence.

The ideal candidate holds an active OT license, has strong documentation skills, and can manage a flexible schedule while delivering compassionate, patient-centered care in the home setting.

Qualifications

• Active licensure as an Occupational Therapist (OT) in good standing.
• Home health experience preferred; strong clinical skills encouraged.
• Excellent communication and clinical documentation abilities.

Responsibilities

• Conduct comprehensive in-home occupational therapy evaluations and deliver targeted treatment interventions.
• Develop and implement individualized care plans designed to maximize patient independence and functional outcomes.
• Educate patients and caregivers on safety practices, adaptive techniques, and proper use of assistive equipment.
• Complete accurate, timely, and compliant clinical documentation for all patient visits.
• Collaborate effectively with the interdisciplinary team to communicate patient progress and adjust care plans as needed.
• Ensure adherence to all clinical, regulatory, and documentation standards.

About the Role

We are seeking a part-time licensed Occupational Therapist (OT) to join our exceptional team in Indiana. We have openings in Lake County (Crown Point, Hobart, Gary, Merrillville, Munster, etc.), Porter County (Valparaiso, Portage, Porter, Boone Grove, South Haven, etc.), and the surrounding areas.
