Job summary

The Judge Group is seeking an experienced Occupational Therapist (OT) to provide skilled home health services across Wabash, Huntington, Grant, Blackford, Madison, and Delaware Counties in IN. This 13-week assignment with extension options runs Monday–Friday, 8:00 AM–4:30 PM EST, with 40 hours per week (36 guaranteed).

The OT will perform evaluations, develop treatment plans, complete OASIS, and coordinate care with the interdisciplinary team, while adhering to CMS and agency standards.

Qualifications

• Current Indiana OT license.
• Current CPR certification.
• Minimum of 1 year of recent Home Health experience.
• Proficiency with OASIS assessments and Homecare Homebase/PointCare documentation systems.
• Valid driver's license and automobile insurance.
• Reliable transportation to travel throughout the assigned coverage area.
• Ability to provide care in diverse home settings and work effectively with patients of all backgrounds.

Responsibilities

• Perform comprehensive occupational therapy evaluations and assessments related to functional status, ADLs, home safety, and adaptive equipment needs.
• Complete OASIS assessments (SOC, Recertification, ROC, Discharge).
• Develop individualized treatment plans with physicians and the care team.
• Deliver OT services within state regulations and agency standards.
• Evaluate body systems for CMS documentation and determine interventions.
• Conduct assessments to evaluate function and rehab needs.
• Implement interventions to improve independence, safety, and quality of life.
• Assess home environments and recommend equipment or modifications.
• Observe and document patient responses and progress toward goals.
• Coordinate care with nursing, therapy, physicians, and caregivers.
• Complete documentation accurately within timeframes.
• Maintain compliance with standards and participate in team initiatives.

Position Summary

We are seeking an experienced and compassionate Occupational Therapist (OT) to provide skilled home health services throughout Wabash, Huntington, Grant, Blackford, Madison, and Delaware Counties. The Occupational Therapist will assess, evaluate, and treat patients with functional limitations impacting activities of daily living, mobility, safety, and independence within the home environment.

The ideal candidate will have recent home health experience, be proficient in OASIS documentation and Homecare Homebase/PointCare, and demonstrate strong clinical judgment, communication skills, and a patient-centered approach to care.

Essential Responsibilities

• Perform comprehensive occupational therapy evaluations and assessments related to functional status, activities of daily living (ADLs), fine motor coordination, home safety, adaptive equipment needs, and other occupational therapy needs associated with the patient's medical condition.
• Complete OASIS assessments, including Start of Care (SOC), Recertification, Resumption of Care (ROC), and Discharge visits.
• Develop individualized treatment plans in collaboration with physicians and the interdisciplinary care team.
• Provide occupational therapy services within the scope of practice defined by state regulations, agency policies, and evidence-based standards of care.
• Evaluate all body systems as required for CMS documentation and determine appropriate occupational therapy interventions.
• Conduct patient assessments utilizing diagnostic and prognostic functional ability testing to evaluate level of function and rehabilitation needs.
• Implement therapeutic interventions designed to improve independence, safety, and overall quality of life.
• Assess home environments and recommend adaptive equipment, environmental modifications, and strategies to enhance patient safety and function.
• Observe, document, and communicate patient responses to treatment, changes in condition, and progress toward goals.
• Coordinate patient care with nursing staff, therapy staff, physicians, caregivers, and other members of the healthcare team.
• Complete all required documentation accurately and within established timeframes.
• Maintain compliance with all federal, state, agency, and accreditation standards.
• Participate in care conferences, team meetings, and quality improvement initiatives as needed.

Coverage Area

Travel throughout the assigned counties to provide home-based occupational therapy services.

Mileage Reimbursement

• Mileage reimbursement is provided between patient visits.
• Mileage is not reimbursed for travel from home to the first patient visit or from the last patient visit back home.

Required Qualifications

• Current Occupational Therapist (OT) license in the state of Indiana.
• Current CPR certification.
• Minimum of one (1) year of recent Home Health experience.
• Proficiency with OASIS assessments and Homecare Homebase/PointCare documentation systems.
• Valid driver's license and current automobile insurance.
• Reliable transportation to travel throughout the assigned coverage area.
• Ability to provide care in diverse home settings and work effectively with patients of all backgrounds and socioeconomic circumstances.
