One Home Health is seeking a part-time Occupational Therapist to serve patients in the Chicago area, delivering in-home care with autonomy and flexibility. The OT will evaluate, plan, and implement interventions to help patients regain independence.

Qualifications

• Active Illinois OT license.
• Current driver’s license and reliable transportation.

Responsibilities

• Perform occupational therapy evaluations in patients' homes.
• Develop individualized treatment plans based on patient goals and clinical needs.
• Provide therapeutic interventions to improve independence, mobility, and daily living skills.
• Educate patients and caregivers on safety strategies, adaptive techniques, and proper use of assistive equipment.
• Collaborate with nurses, therapists, physicians, and other interdisciplinary team members to coordinate care.
• Accurately complete and submit clinical documentation in a timely manner.
• Ensure compliance with agency policies, Medicare regulations, and professional standards of practice.
