Continuum Therapy Partners is seeking an Occupational Therapist to deliver high-quality, patient-centered care on a PRN daytime schedule in Cleveland, OH. You will assess functional needs, craft individualized treatment plans, and collaborate with a leadership team of experienced therapists to maintain consistent care delivery.

The role requires an OT license (state), NBCOT certification, and a master’s/doctorate in OT with preference for 1 year of experience.

Formación

• Master's or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.

Responsabilidades

• Evaluates and develops individualized treatment plans to improve patients' functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.
