Job summary

Continuum Therapy Partners, LLC is seeking a licensed Occupational Therapist to join our PRN team at Heritage of Hudson in Hudson, Ohio. You’ll provide high‑quality, patient‑centered care with flexible daytime PRN shifts and supportive leadership to maintain consistency with our standard of care.

You will evaluate patients, develop individualized treatment plans, document progress, and educate families on equipment and home safety. This role is on-site at Heritage of Hudson, OH.

Qualifications

• Master’s or Doctorate in Occupational Therapy is required.
• Current state OT license and NBCOT certification are required.
• 1 year of OT experience preferred.
• Develops individualized treatment plans to improve functional independence.
• Documents patient progress and modifies plans based on outcomes.
• Educates patients and families on adaptive equipment and home safety.

Occupational Therapist Requirements & Responsibilities

• Master’s or Doctorate Degree in Occupational Therapy required
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required
• 1 year of previous experience as an Occupational Therapist preferred
• Evaluates and develops individualized treatment plans to improve patients’ functional independence
• Documents patient progress and modifies treatment plans based on clinical outcomes
• Educates patients and families on adaptive equipment and home safety recommendations
