Continuum Therapy Partners is seeking an Occupational Therapist to join our team at Heritage Manor in Chisholm, MN. This PRN position focuses on developing and implementing individualized treatment plans to improve patients’ independence, with documentation of progress and outcomes.

The ideal candidate holds a Master’s or Doctorate in Occupational Therapy, a current state license, NBCOT certification, and 1 year of experience.

Qualificações

• Master's or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.

Responsabilidades

• Evaluates and develops individualized treatment plans to improve patients' functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.
