Job summary

Sunnyvale Gardens Post Acute in Sunnyvale, CA is seeking a PRN Occupational Therapist (OTR) to join our rehabilitation team. You will develop individualized treatment plans and provide OT services per physician orders in a resident-centered setting.

We welcome new grads and offer mentorship, flexible shifts, and access to advanced therapy technology, including ACP equipment, OmniVR, and OmniCycle, to support high-quality care.

Responsibilities

• Develop individualized treatment plans and obtain physician approval
• Provide occupational therapy services per physician orders
• Assist nursing staff with training Restorative Aides
• Supervise Occupational Therapy Assistants in accordance with state practice acts
• Communicate patient progress, challenges, and care plans with the interdisciplinary team
• Participate in patient care conferences, utilization review, and rehabilitation meetings as needed

Pay Range: $57–$67 per hour
Schedule: Flexible
