Job summary

Sunnyvale Gardens Post Acute in Sunnyvale, CA is seeking a PRN Occupational Therapist (OTR) to join our rehabilitation team. You will develop individualized treatment plans, obtain physician approvals, and provide OT services per orders.

We offer flexible scheduling, strong clinical support, and mentorship for new graduates. You will supervise OT Assistants and collaborate with nursing and therapy staff to optimize patient outcomes in a resident-centered environment.

Qualifications

• Current and valid Occupational Therapy License (OTR) in good standing.
• Ability to work in a dynamic rehabilitation team.

Responsibilities

• Develop individualized treatment plans and obtain physician approvals.
• Provide occupational therapy services per physician orders.
• Assist nursing staff with training Restorative Aides.
• Supervise Occupational Therapy Assistants in accordance with state acts.
• Communicate progress to the interdisciplinary team.
• Participate in patient care conferences and rehabilitation meetings.
