Job summary

ARC Rehab is seeking a PRN Occupational Therapist (OT) to join Lee Manor Nursing & Rehabilitation in Des Plaines, IL. The OT will evaluate residents, develop plans of care, provide treatments, and document outcomes while collaborating with an interdisciplinary team.

Candidates must hold an Illinois OT license, communicate effectively with patients and families, and be comfortable with computer-based documentation. SNF experience is preferred but not required.

Qualifications

• Must hold an Illinois OT license.
• Good communication with patients, families, and caregivers.
• Comfortable with computers and electronic documentation.
• SNF experience preferred but not required.

Responsibilities

• Perform evaluations and develop plans of care for residents.
• Provide OT treatment and document progress and outcomes.
• Screen residents to identify those who may benefit from therapy.
• Communicate patient status and needs to patients, families, and the care team.
• Direct and supervise assistants as applicable.
• Engage in continuing education and professional development.
• Deliver in-service training and participate in interdepartmental meetings.

Accommodate, Restore & Collaborate (ARC Rehab) is seeking a PRN Occupational Therapist (OT) at Lee Manor Nursing & Rehabilitation in Des Plaines, IL. The job duties include, but are not limited to, the following:

• Performing evaluations/plans of care and providing treatment accordingly.
• Appropriately and effectively completing documentation.
• Screening residents and individuals who may benefit from therapy.
• Communicating patient status and needs to the patients, family, caregivers, or other members involved with patient care.
• Directing and supervising the work of assistants as applicable.
• Expanding and enhancing self, in both clinical and professional knowledge through associations and continuing education.
• Providing in-service training sessions or other educational presentations to members of the interdisciplinary team as needed.
• Supporting and participating in department operations/meetings.

Requirements

• Licensed in the state of Illinois as an Occupational Therapist
• Ability to communicate well with patients, families, supervisors, and colleagues
• Comfortable with computers
• Preferred experience working at a SNF, but we are open to other practice area experience
