Job summary

Louisville East Post Acute is seeking a PRN Occupational Therapist to assess residents within 24 hours of referral and to develop and implement patient-specific treatment plans. The OT will work with the physician and health team, documenting progress and coordinating with supervisors to ensure quality care.

Responsibilities include training Restorative Aides, communicating patient progress, and maintaining accurate medical records.

Develop effective treatment plan and obtain approval for services from referring physician. Treat Residents per the physician treatment plan. Assist nursing department with training of Restorative Aides. . Communicate with supervisor and other health team members regarding patient progress, problem and plans.
