Pleasant Hill Post Acute in Pleasant Hill, CA is seeking an Occupational Therapist to join our rehabilitation team. This role offers a competitive hourly pay range and a patient-centered, collaborative environment.

Qualifications

Current California OT license in good standing.
Strong clinical reasoning, communication, and organizational skills.
Ability to work effectively as part of a multidisciplinary team.

Experience in skilled nursing, post-acute, or rehabilitation preferred.

Responsibilities

Evaluate patients and develop individualized treatment plans based on their needs and goals.
Provide skilled occupational therapy services to improve independence with activities of daily living.
Help patients maximize functional abilities and improve quality of life.
Monitor progress and modify treatment plans as appropriate.
Collaborate with physicians, nursing staff, PT, SLP, and other team members.
Communicate treatment goals and progress with patients, families, and caregivers.
Complete accurate, timely, and compliant clinical documentation.
Participate in interdisciplinary care planning and discharge planning as appropriate.
