Job summary

Homeside Rehab is seeking compassionate Occupational Therapists (OTs) and Physical Therapists (PTs) to join our home health team in Spring Grove, PA.

You will provide individualized, evidence-based therapy services to patients in their homes, with a focus on one-on-one care, schedule flexibility, and a supportive clinical and administrative team.

Qualifications

• Active PA OT or PT license is required.
• Proficient with electronic documentation.
• Strong clinical assessment and interpersonal communication skills.
• Ability to work independently in a home health environment.
• Valid driver’s license and reliable transportation for home visits.
• Pediatric experience preferred.
• Minimum of 1 year of experience in Home Health, Outpatient, Rehabilitation, or SNF settings preferred.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on assessments and patient goals.
• Provide skilled OT or PT interventions within your scope of practice.
• Help patients improve mobility, strength, and functional independence.
• Monitor progress and adjust treatment plans as clinically appropriate.
• Educate patients and caregivers on goals, exercises, safety, and home programs.
• Maintain accurate clinical documentation per company and regulatory requirements.
• Collaborate with the clinical team for coordinated patient care.
• Communicate professionally with patients, caregivers, and the care team.

Homeside Rehab is seeking compassionate and skilled Occupational Therapists (OTs) and Physical Therapists (PTs) to join our home health team in Spring Grove, PA.

In this role, you will provide individualized, evidence-based therapy services to patients in the comfort of their homes. This is a great opportunity for therapists who value schedule flexibility, one-on-one patient care, and a supportive clinical and administrative team.

Job Responsibilities

• Evaluate patients and develop individualized treatment plans based on clinical assessments, functional needs, and patient goals

• Provide skilled Occupational Therapy or Physical Therapy interventions within your professional scope of practice

• Help patients improve mobility, strength, motor skills, functional independence, and overall quality of life

• Monitor patient progress and adjust treatment plans as clinically appropriate

• Educate patients and caregivers regarding treatment goals, home exercise programs, safety, and functional strategies

• Maintain accurate and timely clinical documentation in accordance with company and regulatory requirements

• Collaborate with the clinical team to ensure coordinated and effective patient care

• Communicate professionally with patients, caregivers, office staff, and other members of the care team

Job Qualifications

Job Details

• Job Type: Full-Time, Part-Time, Per Diem

• Pay: $65–$80 per hour

• Schedule: Flexible – choose your own availability

• Day and evening shifts available

• Monday through Friday

• Weekend availability as needed
