Job summary

Terraces of Lake Worth Care Center in Lake Worth, FL is seeking an Occupational Therapist (Per Diem) to join our interdisciplinary team. You will assess residents' physical and cognitive abilities and develop individualized treatment plans to promote independence and safety.

You will implement OT interventions such as ROM, strengthening and cognition exercises, and collaborate with nurses, therapists, and physicians to support residents' goals.

Responsibilities

• Conduct thorough assessments of residents' physical and cognitive abilities
• Develop and implement individualized treatment plans to address residents' goals and needs
• Provide occupational therapy interventions, including but not limited to, range of motion exercises, strengthening and stretching techniques, and cognition and memory exercises
• Collaborate with other healthcare professionals to ensure comprehensive care
• Maintain accurate records of treatment plans, progress notes, and discharge summaries
• Participate in interdisciplinary team meetings to discuss resident progress and needs
