Qualifications

• State Tennessee Occupational Therapist license is required.
• Graduate of an accredited OT program.
• Basic Life Support (BLS) certification, current.

Responsibilities

• The Occupational Therapist provides high-quality, patient-centered care by managing the complete process of occupational therapeutic care of an assigned group of patients, in coordination with the physician and the patient care team.
• Completes patient assessments and participates in developing and implementing individualized care plans, focusing on goal achievement, patient engagement, and positive patient experience.
• Delivers or directs a majority of the patient’s therapy to support patients throughout home health and the continuum of care.
