Job summary

Providence Centralia Hospital in Centralia, WA seeks an occupational therapist to join our patient‑focused care team.

You will provide OT services including assessment, treatment planning and implementation, documentation, and collaboration with the care team. Required: OT degree (Bachelor's or Master's from an accredited program), WA OT license on hire, BLS via American Heart Association, and experience in geriatrics across acute, rehab, and outpatient settings.

Qualifications

  • Washington Occupational Therapist License upon hire.
  • National Provider BLS - American Heart Association upon hire.
  • Clinical experience as an Occupational Therapist in acute care, rehab, skilled nursing, and/or outpatient.
  • Experience in geriatrics.

Required Qualifications:

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
