Position Summary

We have an exciting opportunity to join our team as an Occupational Therapist – Outpatient Ortho – Patchogue.

Job Responsibilities

• Maintain accurate, complete, and timely documentation reflecting the examination, evaluation, diagnosis, prognosis, plan of care, education, and interventions.
• Interact and collaborate with other members of the healthcare team to optimize patient outcomes and healthcare goals; provide on‑site clinical supervision to Occupational Therapist Assistants and Rehabilitation Aides.
• Evaluate patient progress toward functional goals and modify the plan of care accordingly.
• Provide education to patients, families, and clinicians regarding functional status and plan of care.
• Perform objective functional evaluations using appropriate tests and measures, and establish a plan of care with measurable goals.
• Implement appropriate interventions and treatments in conjunction with the plan of care.
• Perform other duties as assigned.

Minimum Qualifications

• Bachelor’s Degree in Occupational Therapy from an ACOTE‑approved program.
• Current and valid NYS Occupational Therapy license and registration or limited permit.
• Current and valid BLS or CPR certification from the American Heart Association (Inpatient facility only).
• Excellent oral and written communication skills.
• Basic computer skills.

Preferred Qualifications

• Master’s Degree.
• Current and valid BLS or CPR certification from the American Heart Association (Outpatient facility only).
• 1–2 years experience in a Hospital (Inpatient) or Orthopedic (Outpatient) Rehabilitation setting.
• Experience in a Neuro outpatient setting.
