Job summary

Linley Park Post Acute, a skilled nursing and rehabilitation facility in Anderson, SC, is seeking a licensed Occupational Therapist (OTR) for PRN coverage. The role focuses on evaluation, treatment planning, teamwork, and supervising therapy staff to improve residents’ independence and quality of life.

You will evaluate residents, develop treatment plans, document progress, and collaborate with a multidisciplinary team.

Qualifications

• Degree in Occupational Therapy (Bachelor’s required; Master’s or Doctorate preferred).
• Active South Carolina OTR license in good standing.
• Experience in skilled nursing or rehab setting preferred.
• Familiarity with PointClickCare and Casamba preferred.

Responsibilities

• Evaluate residents within required timeframes and develop individualized treatment plans.
• Provide therapy services per physician orders and established goals.
• Document evaluations, treatments, and progress notes in compliance with regulations.
• Supervise COTAs, aides, and students in accordance with state practice act.
• Collaborate with the interdisciplinary team on care planning and patient progress.
• Participate in care conferences, utilization reviews, and rehab meetings.
• Educate residents, families, and caregivers on therapy programs and discharge plans.

Position Summary

We are seeking a licensed Occupational Therapist (OTR) to provide high-quality, patient-centered therapy services. This role includes evaluation, treatment planning, team collaboration, and supervision of therapy staff, all focused on improving residents’ independence and quality of life.

Key Responsibilities

• Evaluate residents within required timeframes and develop individualized treatment plans
• Provide therapy services per physician orders and established goals
• Document evaluations, treatments, and progress notes in compliance with regulations
• Supervise COTAs, aides, and students in accordance with state practice act
• Collaborate with the interdisciplinary team on care planning and patient progress
• Participate in care conferences, utilization reviews, and rehab meetings
• Educate residents, families, and caregivers on therapy programs and discharge plans
• Assist with durable medical equipment (DME) recommendations
• Provide staff training and participate in in-services
• Ensure compliance with all state, federal, and payer requirements

Qualifications

• Degree in Occupational Therapy (Bachelor’s required; Master’s or Doctorate preferred)
• Active South Carolina Occupational Therapist (OTR) license in good standing
• Experience in skilled nursing or rehab setting preferred
• Familiarity with PointClickCare and Casamba preferred
• Strong clinical, documentation, and leadership skills
