Job summary

Moraga Post Acute is currently hiring an Occupational Therapist (OTR) on a PRN basis. Starting rate is $58–$63+ per hour in Moraga, CA. The ideal candidate holds a California OTR license and can provide high-quality, resident-centered care.

The OT will evaluate residents, develop individualized plans, supervise COTAs, and collaborate with an interdisciplinary team. Expect flexible scheduling and opportunities to improve ADLs and safety, with involvement in discharge planning and education for

Qualifications

Current California OTR license required.
Strong clinical, communication, and organizational skills.
Ability to work effectively within a multidisciplinary team.
Commitment to resident-centered care.

Responsibilities

Evaluate residents and develop individualized treatment plans.
Provide skilled OT services focused on improving ADLs.
Supervise and collaborate with COTAs.
Coordinate care with physicians, nursing staff, residents and families.
Participate in care conferences, rehab meetings and discharge planning.
Educate residents and caregivers on goals, home exercise programs, safety, and fall prevention.
Recommend adaptive equipment and durable medical equipment (DME).
Maintain accurate, timely, and compliant clinical documentation.
Contribute to quality improvement initiatives and staff education programs.
