Job summary

Sobe Innovative Rehab is seeking a licensed Occupational Therapist to provide outpatient therapy services in a home setting. The OT will help older adults maintain independence and quality of life with evidence/comfort-based care.

This PRN role offers flexibility to select assignments that fit your schedule. Responsibilities include evaluating residents at home, creating and implementing individualized plans, collaborating with physicians and families, and maintaining EMR documentation.

Qualifications

• Bachelor's, Master's, or Doctorate degree in Occupational Therapy from an accredited program.
• Active Florida Occupational Therapist (OT) license.
• New graduates and experienced therapists are encouraged to apply.

Responsibilities

• Evaluate and treat residents in a home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.
