Job summary

Sobe Innovative Rehab in Palm Coast, FL is seeking a licensed Occupational Therapist to provide outpatient therapy services in a home setting. This PRN role offers flexible assignments and the opportunity to support older adults in maintaining independence and quality of life.

You will evaluate and treat residents at home, develop individualized treatment plans, collaborate with physicians and families, and ensure accurate EMR documentation.

Qualifications

• Active Florida OT license required.
• OT degree from an accredited program.
