The Chatham Hills Subacute Care Center is seeking a dedicated Occupational Therapist to join our rehabilitation team. You will assess patients in need of rehabilitative services, develop and implement treatment plans, and educate patients and families on therapy methods.

Responsibilities include supervising Occupational Therapy Assistants, documenting patient progress, and collaborating with the care team to deliver patient-centered care.

Qualifications

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Overseeing and supervising Occupational Therapy Assistants.

Responsabilités

• Assess referred patients requiring rehabilitation services, provide direct patient care, and determine potential for rehabilitation.
• Establish and administer treatment programs with goals based on patient capacity and tolerance under physician direction.
• Document patient condition, record progress, and educate patients and families in therapy methods.
