Job summary

HealthPro Heritage in Lombard, IL is seeking an Occupational Therapist (OTR) for PRN hours to lead a therapy team in a beautifully remodeled retirement community. This role serves residents across Independent Living, Assisted Living and Memory Care, with a Monday–Friday schedule and options for flexible hours.

You will perform patient assessments, develop treatment plans, educate clients and families, document progress, and advocate for community resources.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• valid state licensure as an Occupational Therapist, or license eligible.
• Strong clinical assessment and diagnostic skills; proficient in developing and implementing treatment plans.
• Excellent communication and interpersonal skills with a patient-centered approach.

Responsibilities

• Patient assessment: evaluate clients' physical, emotional, and cognitive abilities using observations and standardized tools.
• Treatment and education: create personalized treatment plans and educate clients and families on improving daily activities with adaptive equipment and environmental modifications.
• Documentation: maintain accurate records of evaluations, progress, and treatment plans, ensuring compliance with standards.
• Advocacy and support: advocate for clients' needs and connect them with community resources and accommodations.

• Physical Requirements: Ability to lift and assist patients as needed. Must be able to stand for extended periods and perform physical tasks related to therapy.
