HealthPRO Heritage is seeking an Occupational Therapist (OTR) for part-time hours in our senior living communities in Belvidere and Rockford, IL. The role focuses on patient assessments, individualized treatment plans, and family education in a collaborative care setting.

Qualifications

• Degree in Occupational Therapy from an accredited institution.
• Valid state licensure as an Occupational Therapist, or license eligible.
• Strong clinical assessment and diagnostic skills; capable of developing and implementing effective treatment plans.

• Ability to lift and assist patients and stand for extended periods.

Responsibilities

• Assess patients' physical, emotional, and cognitive abilities using observations and standardized tools.
• Develop and implement personalized treatment plans; educate clients and families on daily activities and adaptive equipment.
• Maintain accurate documentation of evaluations, progress, and plans; ensure compliance.
• Advocate for clients and connect them with community resources and accommodations.

Qualifications
