Job summary

Patewood Post Acute is seeking a licensed Occupational Therapist (OT) to provide patient-centered therapy services in a 120-bed skilled nursing and rehabilitation facility in Greenville, SC. This full-time OT role emphasizes evaluation, treatment planning, interdisciplinary collaboration, and documentation compliance.

The ideal candidate will hold a South Carolina OT license, have SNF/reha experience, and be proficient with documentation and interprofessional teamwork.

Qualifications

• Bachelors degree in Occupational Therapy is required; Masters or Doctorate is preferred.
• Active South Carolina Occupational Therapist license in good standing
• Experience in skilled nursing or rehabilitation setting preferred
• Familiarity with PointClickCare and Casamba
• Strong clinical, documentation, and communication skills

Responsibilities

• Evaluate residents and develop individualized treatment plans
• Provide skilled occupational therapy services per physician orders
• Document evaluations, treatments, and progress notes in compliance with regulations
• Collaborate with physicians, nursing, and interdisciplinary team members
• Educate residents, families, and caregivers on treatment plans and home programs
• Participate in care planning, discharge planning, and team meetings
• Conduct staff training and in-service education
• Maintain compliance with state practice acts and facility policies

Position Summary
