Job summary

Enhance Rehabilitation is seeking qualified Occupational Therapist candidates to join our SNF team. You will evaluate and treat patients, develop plans of care, and set measurable functional goals to support rehabilitation.

You will supervise Occupational Therapy Assistants, re-evaluate progress, document evaluations and progress notes, and collaborate with facility staff to ensure excellent care and compliance with standards.

Qualifications

• Licensed OT in Illinois or eligible for licensure
• Experience in SNF settings preferred
• Ability to develop and implement individualized treatment plans

Responsibilities

• Evaluate and treat patients in SNF following individualized plan of care
• Develop plans of care and set measurable functional goals
• Re-evaluate patient progress and revise plans of care as needed
• Complete accurate documentation including evaluations, re-evaluations, discharges and progress notes
• Supervise Occupational Therapy Assistants and collaborate with facility staff

As an Enhance Rehabilitation Occupational Therapist, you will be a part of growing team. In this role, you will evaluate and treat patients in a SNF setting following an individualized plan of care.

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
