Job summary

Enhance Rehabilitation is seeking qualified Occupational Therapist candidates to join our dynamic team in a SNF setting. You will evaluate and treat patients following an individualized plan of care, ensuring measurable functional goals are met.

Responsibilities include supervising Occupational Therapy Assistants, documenting evaluations and progress notes, and collaborating with facility staff to deliver excellent patient care and service to patients and families.

Responsibilities

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, addressing concerns and educating them on injury, progress and outcomes
• Re-evaluating patient progress and revising plans of care
• Completing timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes
• Supervising Occupational Therapy Assistants
• Collaborating with facility staff to ensure excellent patient care
• Providing excellent care and customer service to patients and families
