Job summary

Enhance Rehabilitation is seeking qualified Occupational Therapist candidates to join our dynamic SNF care team. In this role, you will evaluate and treat patients in a SNF setting following an individualized plan of care.

You will collaborate with facility staff, educate patients and families, document evaluations and progress, and supervise Occupational Therapy Assistants to ensure high-quality care and measurable outcomes.

Responsibilities

• Evaluates new patients and develops plans of care with measurable goals.
• Treats patients and educates them on injuries, progress and outcomes.
• Re-evaluates progress and revises plans of care.
• Completes timely documentation including evaluations, re-evaluations and discharge notes.
• Supervises Occupational Therapy Assistants.

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
