Centra Healthcare's client in Sunnyvale, CA is seeking an Occupational Therapist (OT) for a part-time, direct-hire role in a Skilled Nursing Facility. The OT will provide skilled services per physician orders, assess patient needs, and collaborate with the care team to achieve goals.

Candidates must have graduated from an accredited OT program, hold a state license or be in licensure process, and maintain CPR certification.

Job Description

A great Skilled Nursing Facility (SNF) is looking for an experienced or newly graduated Occupational Therapist / OT to join their growing team for a direct hire / 'perm' assignment!

• This is a direct hire / 'perm' position.
• Days schedule.
• This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements include

• Must have graduated from an accredited school.
• Current state license as an Occupational Therapist / OT or proactively in the process of the application process for current state licensure as an Occupational Therapist / OT.
• Current CPR certification.
• Provide skilled occupational therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of occupational therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
