Position Summary

A great Skilled Nursing Facility (SNF) is looking for an experienced or newly graduated Occupational Therapist (OT) to join their growing team on a direct‑hire permanent assignment.

Responsibilities

• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions, and potential for rehabilitation.
• Establishing and administering a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the physician.
• Setting realistic and achievable goals for patients, documenting and recording the patient's condition, and educating patients and families in an appropriate occupational therapy method.
• Providing skilled occupational therapy services/interventions in accordance with physician orders.
• Assessing patient needs, planning for, evaluating and modifying care to meet the goals of occupational therapy interventions.
• Collaborating with all disciplines to plan and evaluate team goals for each patient.
