Job summary

Evolve Therapy Services is seeking a compassionate Occupational Therapist to provide in-house therapy services at a Skilled Nursing Facility in Manitowoc, WI. This PRN role will plan, organize, and direct occupational therapy in alignment with regulations and facility goals.

Responsibilities include screening residents, evaluating treatment plans, coordinating with the team, and ensuring accurate documentation and billing. Active state licensure and ongoing continuing education are required.

Qualifications

• Must have an active Occupational Therapy license in practicing state(s).
• Degree in Occupational Therapy.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best intervention for each patient.
• Evaluate residents to inform and develop treatment plans; set short- and long-term goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families, and the interdisciplinary team.
• Establish individual resident therapy schedules with other team members.
• Deliver therapy according to the plan and ensure proper documentation and billing for occupational therapy services.
• Perform all other duties as assigned to effectively treat patients.
• Oversee COTA as needed and ensure documentation and treatments meet clinical excellence and reimbursement practices.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient

• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals

• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members

• Establish individual resident therapy schedules in cooperation with other team members

• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services

• Perform all other duties as assigned and as required to effectively treat patients

• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy

• MUST have an Active Occupational Therapy license in practicing state(s)

• Ongoing continuing education and professional development to maintain current licensure and certification
