Job summary

Evolve Therapy Services in Kenosha, WI, seeks an Occupational Therapist for an in-house Skilled Nursing Facility. This PRN role offers consistent or casual hours to fit your schedule and focuses on delivering high-quality therapy to residents.

You will evaluate residents, plan and implement OT services, supervise COTA as needed, document and bill accurately, and collaborate with an interdisciplinary team to support resident goals and quality of care.

Qualifications

• Degree in Occupational Therapy is required.
• Active OT license in the practicing state(s) is required.
• Ongoing continuing education and professional development to maintain licensure.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: PRN (Consistent or Casual Hours available)

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient

• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals

• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members

• Establish individual resident therapy schedules in cooperation with other team members

• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services

• Perform all other duties as assigned and as required to effectively treat patients

• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy

• MUST have an Active Occupational Therapy license in practicing state(s)

• Ongoing continuing education and professional development to maintain current licensure and certification
