Evolve Therapy Services is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in South Range, WI. This PRN role focuses on delivering quality OT services and coordinating with an interdisciplinary team to support resident care.

Responsibilities include screening, evaluation, treatment planning, scheduling, documentation, and supervision of COTA. Active licensure in Wisconsin is required and ongoing professional development is encouraged for quality patient care.

Qualificações

• Degree in Occupational Therapy.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure.

Responsabilidades

• Conduct regular screening of residents to determine best intervention for each patient.
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned to effectively treat patients.
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient

• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
