Job summary

Evolve Therapy Services, LLC is seeking a compassionate Occupational Therapist for an in-house Skilled Nursing Facility in Fort Atkinson, Wisconsin. This PRN role focuses on planning, organizing, and directing OT services to ensure high-quality resident care in compliance with standards and regulations.

You will screen residents, develop treatment plans, coordinate schedules with the team, and supervise COTA as needed, while ensuring proper documentation and billing practices.

Qualifications

• Must have an active Occupational Therapy license in practicing state(s).
• Continuing education and professional development to maintain licensure.

Responsibilities

• Conduct regular screenings to identify best intervention for each resident.
• Evaluate residents to inform and develop treatment plans with short- and long-term goals.
• Communicate findings and plans to residents, families, and the interdisciplinary team.
• Establish individual therapy schedules in coordination with team members.
• Document and bill occupational therapy services per plan.
• Perform duties to effectively treat patients and as assigned by supervisor.
• Oversee COTA and ensure documentation and treatments meet clinical and reimbursement standards.

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

PRN

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
