Job summary

Sigma Health Rehab is seeking a full-time Occupational Therapist (OT) to work in a Skilled Nursing Facility. The OT will evaluate residents, develop individualized treatment plans, and help patients regain independence in daily activities.

New graduates with relevant experience are encouraged to apply; NY license is required. Slate Valley Center for Rehabilitation and Nursing, located in Granville, NY, supports professional development and state-of-the-art therapy practices.

Qualifications

• Graduate of an accredited OT program.
• Current New York State OT License.
• Experience in a skilled nursing or geriatric care setting preferred; new grads welcome.

Responsibilities

• Evaluate residents and develop OT treatment plans tailored to individual needs.
• Select activities to help residents perform daily tasks independently.
• Observe, assess, and document OT progress with detailed reports.
• Protect resident confidentiality and process information securely.
• Maintain cooperative relationships within the healthcare team.

OT Requirements Include:

• Graduate of an accredited Occupational Therapy Program
• Current New York State OT License
• Experience in a skilled nursing or geriatric care setting is preferred, but new graduates with relevant experience are encouraged to apply

Key Responsibilities:

• Evaluate Residents & develop an OT treatment plan that will meet their individual needs.
• Choose activities that will help the residents to carry out their daily activities independently.
• Observe, assess, and document residents' occupational therapy progress by preparing detailed reports.
• Maintain resident confidence by monitoring confidential information processing.
• Maintains a cooperative relationship among healthcare team
