Sigma Health Rehab is seeking a full-time Occupational Therapist (OT) to join our Skilled Nursing Facility team in Monsey, NY. You will evaluate residents, develop and implement OT treatment plans, and support recovery of daily living activities.

We value licensed New York OTs, with SNF/geriatric experience preferred. New grads with relevant exposure are encouraged to apply, and you will work with a supportive team using state-of-the-art equipment.

Qualifications

• Graduate of an accredited occupational therapy program.
• Current New York OT license required.
• Experience in skilled nursing or geriatric care preferred; new grads with relevant experience encouraged to apply.

Responsibilities

• Evaluate residents and develop individualized OT treatment plans.
• Select activities to help residents perform daily activities independently.
• Observe, assess, and document progress with detailed reports.
• Maintain resident confidentiality in information processing.
• Collaborate with healthcare team to support rehabilitation goals.

Key Responsibilities:

• Evaluate Residents & develop an OT treatment plan that will meet their individual needs.
• Choose activities that will help the residents to carry out their daily activities independently.
• Observe, assess, and document residents' occupational therapy progress by preparing detailed reports.
• Maintain resident confidence by monitoring confidential information processing.
• Maintains a cooperative relationship among healthcare team
