Qualifications

• Graduate of an accredited occupational therapy program.

Responsibilities

• Initiate OT treatments per physician orders and maintain communication with physicians.
• Complete and maintain evaluations, progress notes, MDS documentation, and discharge summaries.
• Collaborate with interdisciplinary healthcare team to address residents’ rehabilitation and functional maintenance needs.
• Participate in Resident Care Planning meetings and attend in the absence of the Director of Occupational Therapy when needed.
• Maintain statistical records of resident treatments and therapy outcomes.
• Assist with in-service education and training programs upon request.
• Record, update, and monitor resident care plans.
• Support residents in achieving their highest level of independence and functional ability.

As a member of the rehabilitation team, you will help ensure the health, well-being, independence, and quality of life of residents through comprehensive occupational therapy services.

This position includes a schedule of weekdays, with some weekend and holiday coverage.

Key Responsibilities

Professional maturity, diplomacy, and the ability to work collaboratively with residents, families, and staff

Compassionate and patient-centered approach to care

Ability to work effectively with geriatric and chronically ill residents

Commitment to maintaining established standards of clinical excellence
