Twenty80 LLC in Ticonderoga, NY is seeking a licensed Occupational Therapist for a full-time role in a skilled nursing facility. You will provide comprehensive OT services to residents, initiate skilled treatments per physician orders, and document progress in a collaborative rehab team environment.

Requirements include graduation from an accredited OT program and NY state licensure; two years of geriatric/subacute experience is preferred.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current NY state Occupational Therapy license required.
• Two years of occupational therapy experience preferred.
• Interest in geriatric/subacute populations with a team approach.
• Ability to supervise and direct others.
• Strong verbal and written communication skills.

Responsibilities

• Initiate skilled OT treatments per physician orders and maintain physician communication.

• Attend Resident Care Planning meetings and support care plans.
• Maintain records of treatments and therapy outcomes.
