Concept Rehab, a therapist-owned rehabilitation company, seeks an Occupational Therapist in Port Huron, MI to join our skilled nursing facility team. Full-Time or Part-Time opportunities offer a supportive environment and meaningful patient impact.

You'll evaluate patients, develop treatment plans, collaborate with nursing and therapy colleagues, and document care accurately while promoting independence and quality of life for residents.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Michigan OT license or eligibility.
• Strong communication and interpersonal skills.
• Passion for helping patients achieve independence.
• Previous skilled nursing or rehabilitation experience is a plus.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide OT services to help patients improve independence and daily living skills.
• Monitor patient progress and adjust treatment approaches as needed.
• Collaborate with nursing staff, physical therapists, speech therapists, and other healthcare professionals.
• Complete accurate and timely clinical documentation.
• Educate patients, families, and caregivers on therapeutic techniques and strategies.
