Job summary

Vivie is seeking an Occupational Therapist to join our Home Health team in Minnesota. You will deliver compassionate, client-centered therapy, develop individualized treatment plans, and support independence and safety for clients in their homes.

Responsibilities include direct care, accurate documentation in our EMR, collaboration with COTAs and HHAs, and ensuring regulatory compliance while maintaining high standards of care across regions.

Qualifications

• Minnesota OT license is required.
• CPR certification required.
• Valid driver's license and access to insured vehicle.
• Willingness to float across regions.
• Experience in Home Health or rehabilitation settings preferred.
• English proficiency to communicate with clients and team.

Responsibilities

• Provide Direct Care - evaluate clients and develop personalized treatment plans with goals.
• Maintain accurate client records and billing using Vivie EMR.
• Communicate effectively with clients, families, and the interdisciplinary team.
• Ensure regulatory compliance with supervision visits and documentation.
• Other duties as assigned and supportive care as needed.

Schedule: PRN - Minimum 3 shifts per month

Pay Range: $36.55/hr. to $50.70/hr. (hourly, non-exempt) based on qualifications, experience, and location.

As an Occupational Therapist in Home Health, you’ll be at the heart of our community, delivering compassionate and personalized occupational therapy services. Whether providing direct client care, developing individualized treatment plans, you’ll play a key role in enhancing clients' independence, safety, and overall quality of life.

As An Occupational Therapist, You Will

• Provide Direct Care - Evaluate clients by obtaining and interpreting subjective and objective data. Develop personalized treatment plans with short-term and long-term goals, ensuring they align with physician orders. Maintain high standards of care while adapting to different environments and client needs.


• Maintain Accurate Client Records and Billing - Document all services accurately and timely using Vivie’s EMR system. Demonstrate a thorough understanding of all payer sources and authorization requirements. Document collaboration with COTAs and perform supervisory visits for HHAs as per regulatory guidelines.


• Communicate Effectively and Collaboratively - Maintain clear and compassionate communication with clients, families, referral sources, physicians, and the interdisciplinary team. Utilize exceptional verbal and written communication skills to ensure cohesive and effective care delivery.


• Ensure Regulatory Compliance - Complete and document supervisory visits with OTAs and CNAs in accordance with state and regulatory guidelines.

This Job Also Requires

• Current Occupational Therapist (OT) license in the state of Minnesota.


• CPR certification required.


• Valid driver’s license and access to an insured personal vehicle.


• Willingness to float across regions as needed.


• Ability to pass state mandated background check.


• Strong clinical skills in occupational rehabilitation and skilled therapy services.


• Proficiency in reading, writing, and speaking English to communicate effectively with clients, families, and team members.


• Experience in Home Health, acute care, or rehabilitation settings preferred.


• Physical capability to perform all job duties.
