Job summary

Ethos is seeking an Occupational Therapist to deliver home health services, evaluating clients, designing personalized treatment plans, and ensuring safety and independence. You will document care in the EMR, collaborate with the care team, and support regulatory compliance across regions.

Flexible scheduling, travel across regions, and opportunities for professional development are provided. A valid MN/ND OT license and CPR certification are required.

Qualifications

• Current OT license in MN and ND is required.
• CPR/BLS certification is required.
• Valid driver's license and access to insured vehicle.
• Experience in Home Health, acute care, or rehab settings preferred.

Responsibilities

• Provide direct occupational therapy care and develop individualized treatment plans.
• Maintain client records accurately in Vivie EMR and assist with billing as needed.
• Communicate effectively with clients, families, and interdisciplinary team.
• Ensure regulatory compliance through supervision visits with OTAs and HHAs as required.
• Other duties as assigned as part of home health therapy.

As an Occupational Therapist in Home Health, you’ll be at the heart of our community, delivering compassionate and personalized occupational therapy services. Whether providing direct client care, developing individualized treatment plans, you’ll play a key role in enhancing clients' independence, safety, and overall quality of life.

As An Occupational Therapist, You Will

• Provide Direct Care - Evaluate clients by obtaining and interpreting subjective and objective data. Develop personalized treatment plans with short-term and long-term goals, ensuring they align with physician orders. Maintain high standards of care while adapting to different environments and client needs.
• Maintain Accurate Client Records and Billing - Document all services accurately and timely using Vivie’s EMR system. Demonstrate a thorough understanding of all payer sources and authorization requirements. Document collaboration with COTAs and perform supervisory visits for HHAs as per regulatory guidelines.
• Communicate Effectively and Collaboratively - Maintain clear and compassionate communication with clients, families, referral sources, physicians, and the interdisciplinary team. Utilize exceptional verbal and written communication skills to ensure cohesive and effective care delivery.
• Ensure Regulatory Compliance - Complete and document supervisory visits with OTAs and HHAs in accordance with state and regulatory guidelines.

This Job Also Requires

• Current Occupational Therapist (OT) license in the state of Minnesota and North Dakota.
• CPR certification required.
• Valid driver’s license and access to an insured personal vehicle.
• Willingness to float across regions as needed.
• Ability to pass state mandated background check.
• Strong clinical skills in occupational rehabilitation and skilled therapy services.
• Proficiency in reading, writing, and speaking English to communicate effectively with clients, families, and team members.
• Experience in Home Health, acute care, or rehabilitation settings preferred.
• Physical capability to perform all job duties.
