Job summary

CommuniCare Family of Companies seeks a PRN Occupational Therapist to join the in-house rehab team at Forest Hills Healthcare Center in Cincinnati, OH. The OT will evaluate residents, develop and implement treatment plans, supervise COTAs and participate in interdisciplinary care planning.

Opportunity to work across multiple locations within the CommuniCare family, with a focus on resident-centered care and collaboration with family and care teams.

Qualifications

• Bachelor's or Master's Degree in Occupational Therapy from an accredited program.
• Hold a valid or pending State OT license.

Responsibilities

• Perform residents' occupational needs evaluation under physician order and develop a treatment plan.
• Implement treatment plan through direct treatment and educate staff involved in the plan.
• Provide supervision for COTAs.
• Attend or lead interdisciplinary meetings to relay findings and make recommendations.
• Treat at multiple locations to serve diverse resident needs.

Qualifications/Must Have’s

• A Bachelor's, Master’s Degree in Occupational Therapy from an Accredited Program.
• Hold a valid or pending State Occupational Therapy License

Responsibilities include, but are not limited to

• Perform patient’s/resident’s occupational needs evaluation under physician order and develop a treatment plan in accordance with patient’s needs.
• Implement treatment plan through direct treatment, as well as education of treatment staff including therapists and other members of team involved in treatment plan.
• Provide necessary supervision for COTAs.
• Attend/Lead interdisciplinary meetings as appropriate to relay clinical findings and make recommendations to other members of rehab team, including family education and consultation.
