Job summary

CommuniCare Rehab, a member of the CommuniCare Family of Companies, has a full-time Occupational Therapist opening at Grande Pointe Healthcare Center in Richmond Heights, OH. This in-house role focuses on resident evaluation and creating individualized treatment plans that improve functional outcomes.

The ideal candidate holds a Bachelor's or Master's in OT with an active or pending state license, and will supervise COTAs while collaborating with an interdisciplinary rehab team.

Qualifications

• Bachelor's or Master's in Occupational Therapy from an accredited program.
• Active or pending State OT license required.

Responsibilities

• Perform residents' occupational needs evaluation under physician order and develop an individualized treatment plan.
• Implement treatment plan through direct therapy and educate treatment staff and team members.
• Provide supervision for COTAs.
• Attend and lead interdisciplinary meetings to relay findings and make recommendations.
• Opportunity to treat at multiple locations to meet residents' diverse needs.

Qualifications / Must Have’s

• A Bachelor's, Master’s Degree in Occupational Therapy from an Accredited Program.
• Hold a valid or pending State Occupational Therapy License

Responsibilities

• Perform patient’s/resident’s occupational needs evaluation under physician order and develop a treatment plan in accordance with patient’s needs.
• Implement treatment plan through direct treatment, as well as education of treatment staff including therapists and other members of team involved in treatment plan.
• Provide necessary supervision for COTAs.
• Attend/Lead interdisciplinary meetings as appropriate to relay clinical findings and make recommendations to other members of rehab team, including family education and consultation.
