Job summary

The Phoenix Center for Rehabilitation and Nursing is seeking a Per Diem Occupational Therapist (OT) to provide diagnostic and therapeutic services to evaluate, treat, document, and facilitate care for residents with impaired activities of daily living due to disease processes.

You will participate in interdisciplinary team meetings as required and assist the Director of Nursing Service to develop ADL decline prevention policies and procedures to support resident independence and safety.

Qualifications

• Must have excellent verbal and written communication and organizational skills.
• Ability to communicate with residents, families and other healthcare professionals.
• Ability to define problems, collect data, establish facts, and draw conclusions.
• Must have at least two years of related expereince
• Must have a current Occupational Therapist license.

Responsibilities
