Job summary

Adaptive Home Health is seeking an Occupational Therapist to deliver in-home assessments and skilled visits in Detroit. This field-based role offers part-time flexibility, with visits scheduled based on availability, weekdays and weekends as needed.

You will build and execute plans of care centered on ADLs/IADLs, safety, fall prevention, and functional independence, while coordinating with the care team and documents promptly. Detroit-based opportunity with strong growth potential.

Qualifications

• Must have active Michigan OT license in good standing.

The mission of this role

Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.

Objectives

• Carry a high-quality caseload
• Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.
• Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.
• Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.

What you'll own

• Complete in-home OT evaluations and deliver skilled OT visits.
• Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence.
• Recommend and train on adaptive equipment and home modifications as needed.
• Coordinate with nursing and the broader care team when condition or needs change.
• Document promptly and thoroughly — defensible, efficient, and consistent.

Must-haves

• Active Michigan OT license (in good standing)
• Strong clinical judgment and comfort working independently in the field
• Comfort escalating changes in condition to RN/MD and documenting rationale clearly
• Strong communication and documentation
• Reliable follow-through on orders, frequencies, and patient goals
• Valid driver’s license, auto insurance, and reliable transportation and comfort with daily travel across assigned territory.
• Able to carry/transport basic therapy equipment as needed
• Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education)

Nice-to-haves

• Prior home health experience
• Deeper experience in high-signal OT populations (neuro, CVA, fall risk, dementia/caregiver training)
• Strong organization and time management
