Adaptive Home Health is seeking an Occupational Therapist (OT) to provide in-home evaluations and skilled visits across the assigned territory. The role is field-based, full-time, with Monday–Friday hours plus occasional weekends as needed.
You will build plans of care centered on ADLs/IADLs, safety, and independence, while training on adaptive equipment and home modifications. You will coordinate with nursing and the care team and document promptly to support patient outcomes.

The OT role is the front line of that mission: you restore function in the home, build safe routines, reduce avoidable decline, and help patients regain independence in ADLs/IADLs.

Our charting product is designed to reduce documentation burden and keep visits moving, and our scheduling product (launching soon) will make it easier to coordinate visits, reduce last-minute changes, and protect clinician time.

Complete in-home OT evaluations and deliver skilled OT visits.
Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence.
Recommend and train on adaptive equipment and home modifications as needed.
Coordinate with nursing and the broader care team when condition or needs change.
Document promptly and thoroughly - defensible, efficient, and consistent.

Must-haves
Active Colorado OT license (in good standing)
Strong clinical judgment and comfort working independently in the field
Comfort escalating changes in condition to RN/MD and documenting rationale clearly
Strong communication and documentation
Reliable follow-through on orders, frequencies, and patient goals
Current driver's license and comfort with daily travel across assigned territory
Able to carry/transport basic therapy equipment as needed
Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education)
Nice-to-haves
Prior home health experience
Homecare Homebase (HCHB) experience (or similar home health EMR)
Deeper experience in high-signal OT populations (neuro, CVA, fall risk, dementia/caregiver training)
Strong organization and time management
